Preparation Part 1 of 5 - You and your partner will require referrals from your GP or specialist. Ask your GP to make your referral indefinite to avoid having to obtain new referrals. Referrals from specialists are only valid for 3 months.
- Please bring any test results with you on the day of your appointment.
- Complete the medical history questionnaire and submit on line or bring a hard copy with you to your appointment. This form requires medical history information from both partners. If you submit online, you will need to sign the document at your appointment.
- If your GP or specialist has not ordered a semen analysis or screening tests for you and your partner, these will be arranged for you. Basic tests include: Males: semen analysis, Hep. B & C & HIV; Females: Rubella, Hep. B & C & HIV. These tests are part of the basic preparation for pregnancy.
